How to Get Started
with Richmond Spine
If you’ve been referred to us by your provider, welcome, we’re honored to join your care team. From your very first interaction, our aim is to ensure your experience is smooth and supportive.
Let's Get Started
To help us make your referral and intake process as smooth as possible, we ask that you complete a few important steps. By following the steps outlined below, you’ll ensure we have all the necessary information to move forward with your care quickly and efficiently. Thank you for partnering with us as we get started on your treatment journey.
Step 1: Complete Your Intake Forms (You have 3 options)
Option 1:

Please take a moment to fill out our New Patient Health Intake Form so we can prepare for your visit.

Follow the prompts and complete all the information with as much detail as possible.

Completing your forms online is the fastest and most convenient way to ensure we have all the information needed for your visit.
Option 2: Print and Drop Off Form

Download and print the intake form

Fill out the forms at your convenience.

Drop off the completed paperwork at any of our office locations at least 2–3 days before your scheduled appointment.
Option 3: Complete Forms In-Office (May Cause Delays)

If you are unable to complete the forms in advance, please arrive at least 1 hour before your scheduled appointment time to fill out your paperwork in our office.

Please note: Completing forms in-office may result in longer wait times and could delay your visit.
Step 2: Prepare for Your Visit

Bring a valid photo ID and your insurance card.

Bring a list of current medications.

If applicable, bring any referral forms or authorizations
